Individuals living with an HIV+ or AIDS diagnosis have access to multiple resources and support groups to improve their wellness, including interventions and workshops.

Access to BAC's services and programs
Health Homes
Clients can benefit from our Health Homes programs if they are on a Medicaid plan and have two or more chronic conditions, have one chronic condition and are at risk for a second, have one serious and persistent mental health condition. The purpose of this program is to help reduce visits to the emergency room and time spent in the hospital while increasing their access to their routine primary care. Our Care team provides high quality prevention and health screenings to clients while supporting their medical needs.
